![]() ![]() ![]() The patient’s Electronic Health Records (EHR) were then thoroughly reviewed by a pharmacist to assess for the presence of any bleeding events whose root cause could be traced to the use of the anticoagulants. A sample of 60 cases (∼12%) was randomly selected from the cohort for chart review. A total of 518 potential anticoagulant-related bleeding ADR cases were identified. With these NDCs and ICD-10 codes, we searched the MHS medical data repository to identify patients who had at least one anticoagulant dispensed within 30 days of a 2019 outpatient visit associated with a bleeding event. A comprehensive list of ICD-10 codes that categorize bleeding events and thus could potentially be used to code for a bleeding ADR from an anticoagulant was also selected. Methods: National Drug Codes (NDCs) for anticoagulants - specifically warfarin and direct-acting oral anticoagulants - were selected using a customized drug-chooser embedded within the Pharmacovigilance Defense Application System developed by the Defense Health Agency Pharmacovigilance Center. ![]()
